Yes! Legacy Behavioral Health (LBH) is a public safety net provider, serving individuals who may be low income or have little to no financial resources. Services are provided by LBH to all individuals regardless of ability to pay. For individuals who are uninsured or underinsured, our services are provided on a sliding fee scale based on an individual’s adjusted monthly income. Your first visit will include a financial assessment to help determine which costs, if any, may apply to the services you will receive. In addition, Legacy Behavioral Health provides services to individuals with state and federal benefits such as Georgia Medicaid, Medicare, and Medicare Advantage plans.
Legacy Behavioral Health does accept most in-network private insurances that provide mental health/substance use disorder benefits. Each insurance plan has unique coverage criteria, copays and deductibles. Please bring your health insurance card with you for each visit. Our clerical staff will verify your specific coverage before or during your first visit and will inform you of the anticipated cost. Prior to your first visit, we urge you to contact your health insurance carrier’s customer service line to determine the level of mental health/substance use disorder benefits available to you. Out of pocket costs will vary significantly between insurance carriers and plans and are subject to your insurance plan deductibles, copays, and/or coinsurance amounts. All out of pocket costs are due at the time of service.
Legacy Behavioral Health accepts most major policy providers including Anthem, Ambetter, Amerigroup, Blue Cross Blue Shield, Aetna, Champ VA, Cigna, Humana, Georgia Medicaid, Medicare, Peach State Health Plan, Tricare, UMR/United Health Care, VA Medical. To determine if Legacy can accept your private insurance, please speak to the clerical staff or a Revenue Cycle Specialist prior to your first visit.
Automated payment plans are available to LBH clients by contacting the phone number on the LBH statement or by speaking to a member of the clerical staff.
Family Violence Intervention Program, Anger Management Program, and Self-Pay Group are not billable to any state/federally funded or private insurance. These services will not be assessed to the sliding fee scale and will not be claimed to insurance. Associated fees for the listed services above are due at or before the time of service. For those who are uninsured and covered under the state contracted fund: Laboratory Fees, Pharmacy Fees, Tuberculosis Test, and Drug Screens are not applicable to the sliding fee scale and the individual served will be assessed the full fee of the service.
